1. List items currently missing from the questionnaire that is needed to achieve the stated survey objectives.

1. Demographic factors like sex of the child, Place of birth (Health facility or Home), Birth order, Residential (Urban or rural), religion.

2. Reason for partially vaccinated & unaccinated childrens 

3. Paternal factors like economic, Educational, and other social factors
4. Disability factors (Birth anomalies or congenital problem),
5. Vaccination strategy factors used for vaccinations: - Routine or campaign reached through (Fixed, Outreach, and mobile)
6. Missing questions for MCV 2, BCG and IPV is missing for caregivers
Other factors that enhancing for good vaccinations.

2. List questionnaire items currently unrelated to achieving the stated survey objectives.

1. Data of  yellow fever vaccine,

2. Rota vaccine questions,
3. Other questions may be indirectly related to factors affecting vaccination or under vaccinations like AEFI, card replacement, Migration history or Mobility Question around Routine Immunizations but it is not directly related to the survey objective.

3. Recommend how to improve the alignment between questionnaire and the stated survey objectives.

Strongly needed to remove all unnecessary questions that are totally non relevant to answer the survey Objective like yellow fever vaccine, Rota vaccine questions. No. of questionnaire should be in order.

Demographic variables – like sex of child, Birth order, Rural / urban should be added on it.


4. What steps would you take to ensure that interviewers ask the questions in the same standardized manner?

1. Questions should be arranged in logical variable category, understandable, mot misleading and unnecessary probing.
2. Should have clear consent form
3. Develop SOPs for interviewer & supervisor
4. Deliver training that fully prepares survey data collections that bear trustworthy finding for all interviewer and supervisor theoretical at room and field in the real setups.
5. Translate the questionnaire to local. And back to original to check the consistency of the questionnaire
6. A pre-test in real setups with similar area, but out of the selected study cluster to assess the clarity, understandability, and completeness and revise the questionnaire based on the finding of the pre-test.
7. Develop Instruction and SOP for work under the survey.
8. Need close follow up and strict supervision of the data collection process.

5. What factors may contribute to ascertainment bias in this survey?


Violating survey SOPs and guiding procedures for data collections.
Misleading by unnecessary probing.
Recording problem and data entry error
Non-standardized, out dated questionnaire having too many variables.

No field training of surveyor No monitoring by independent agency.Non- availability of cards in majority , No or poor follow up.
